Registration

The following forms must be filled out along with a $50 non-refundable costume deposit and dropped off at the studio to hold a dancer's spot. We ask the first month or session payment be paid before the dancer's first dance class.

Registration Form

The information necessary for us to register your child is asked for on this form. You'll need your child's health card number as well as contact information.

Media Release Form

This form must be mandatorily filled out. During our dance year, pictures are taken around the studio, during Picture Day and video is taken during our Year End Recital!

Snack Account Agreement

This form is optional. Water and snacks are in the waiting area available for purchase. By signing this form, you are giving us consent to open a "Snack Account," for your child. Balances are given out monthly, and are to be paid in a timely manner.

Forms Completed? Follow These Steps to Finish Registering!

1. Call The Studio (leave a message), and find out when we are available to assist you.

2. Come into the studio with your completed forms and pay the first month/term payment, along with your first $50 costume deposit.

3. We will tell you what day and time the student's dance class is.
